The Health 2.0 is made for all professionals concerned with health 2.0 issues (Web 2.0 technologies applied to Health), in particular : the professionals of new technologies and Internet, new media and communication professionals, health professionals, hospital decision makers, the healthcare industry, insurance,
mutual insurances, banks, minister's advisers and members of public institutions. There were 540 participants at this first edition of the Health 2.0 Europe held in Paris. Last edition held in San Francisco in October 2009, brought together 900 participants. San Francisco remains the most important Health 2.0 Conference of the year.

	Summary of key steps:

On March 7, 2010 Barbara Hanke, an active member of our Medical Committee, had the excellent idea of sending me an information by e-mail about the Health 2.0 conference in Paris, taking place just one month later on April the 6 and 7. She thought that it might interest us.

On March 8, 2010 , I decided to contact directly Denise Silber, manager of the organization collaborating with the two american co-founders, for this first european edition of the Health 2.0 Conference held in Paris. I requested to meet her before the Health 2.0 in order to present her our Internet projects in the
-e-health field, and considered possibilities for the Mood Institute to participate to this event, which is the first showcase and
the international reference for subjects in relation with Internet technologies and mobile telephony applied to health.

On March 13, 2010 Denise Silber accepted to meet me, three weeks before the conference, while the program was already printed, and the organizers were finishing to prepare this international event, organized for the first time in Europe! After 30 minutes of convincing explanations, Denise Silber concluded that our projects were interesting and that there was an opportunity for us to present our e-Healthcare tools during the Launching! session which would present the latest Health 2.0 technologies. Indeed, a company could withdraw from the session. In this case, the application of Mood Institute could be accepted with other candidates who already applied for the Launch! a few months ago and were kept on the waiting list.

On March 16, 2010 , I made a first demonstration to Denise Silber about our mood monitoring tools proposed in our new Members Area. The final decision should be taken with the arrival of the two other American organizers Matthew Holt and Indu Subaiya (co-founders of Health 2.0).

On March 25, 2010 , this time I made a second demonstration. This time the 3 organizers are finally gathered. However they informed me that the only imperative was that the all the websites listed had to be open to the public the D-day of the conference. It meant that no test version of the Members Space would be accepted. Whereas we still had planned for several weeks of work before opening the Members Space on the website ... In spite of the deadline, I decided to confirm that the Members Space of the Mood Institute would be ready for April 7, day of the Health 2.0 conference and launch! session.

On March 29, 2010 , the organizers made their choice among dozens of applications: it was ultimately the Mood Institute which was chosen to participate in "launch Panel" on 7 April ... It was an great joy, but quickly caught up by a lot of pressure because we had only eight days to do what we had planned to do within 3 or 4 weeks ...

On the night from 6 to 7 April 2010 , I switch to production and put Members Space of the website online at 3:04 am ...

	On April 7, 2010 12:30 ... beginning of the "Launch!"session

"A presentation of very new "Health 2.0" technologies "

It is 1:30 p.m. it was my turn to go up on the podium in front of hundreds of people. After my official announce of the opening of the Members Space, I started my demonstration of our e-Healthcare and mood monitoring tools on our website.

It is 2:00 p.m. the public voted and results were published ... with 29% the Mood Institute was elected the Best company of the Launch! session ...

A new step in the evolution and development of the Mood Institute.

	Thanks once again to :

Denise, Indu and Matthew, the organizers of the Health 2.0 who accepted me as a last minute’s outsider and gave me my chance.

Barbara who had the very good idea to inform me about the existence of this conference

and of course thank you to all patients and friends who had been very kind to support and encourage me, by email, sms or phone, especially in the days preceding my speech of April 7

Conclusion: I've often repeated to myself my favorite phrases: "Who asks for nothing, has nothing", "100% of the winners
have tried", "failure is primarily a proof that we have tried"...

Wink? : On the night from 6 to 7 April 2009 at 3:30 am, I put public space of the Mood Institute Website online for the first time ... Exactly one year later, day to day, in the night from 6 to 7 April 2010 at 3:04
a.m. I put the Members Space online ... I decide that 7 would be my favorite number! ...

"It suddenly clicked in my head!" by Frederick

In a television serie, I heard someone who said that he suffered from bipolar disorders at a session of group therapy. It suddenly clicked in my head !

I grabbed the dictionary, opened the internet connection, and finally, after visiting several sites, I discovered the Mood Institute website. I opened a topic, I think it was on professional re-employment, I rode the heading and the comments.

I recognized myself in the description of mood disorders. I left a comment in a few lines (I put more than one hour to write ...) that finally had nothing to do in here but it was too late, the “click” was made ... then I disconnected my computer.
The same evening I had a very cordial mail from the founder, Laurent COUSSIRAT, which made me feel better.

Since then, I decided to take myself into control and consult a specialist in mood disorders. I know that this is essential to take my place in the society to recover equilibrium and find my place which should be probably somewhere. I already have visited a large part of the website and I am now reassured that I am not alone and that so much listening, help and support could be proposed to me.

Special thank to Laurent COUSSIRAT again for his humanity, his involvement and support in my all necessary steps.

"I found your site ... and I cried while I was reading it ..." by DM

I found your website - after many days (months? Years?) of research ... and I cried while I was reading it ... I cried because I have finally glimpsed a semblance of hope ... Torn by a disease that prevents me from finding an anchor on Earth, I skimmed off Internet in research of a solution, a place where my numerous talents, (judging by what I am
told...) talents which could find an application that allows me to survive.

And your website is the first one where I found such a researched hope : to live with my work in staying what I am. Because that’s the first website that deals with the individual affected by "a strange illness", and considers him like a PERSON.
Full member of the society, bearing my own ressources, I’m an economic player ...

I like your website because it clearly announces its goals: it proposes a program for re-employment in its core objectives. You also propose appropriate monitoring tools (can we imagine a mood stabilization without a socioprofessional stabilization?).
It is also one of the few sites that recognizes and shows to advantage the artistic talent as a vector of rehabilitation.
It is also the first site which already establishes - already by its name! - an open-mindedness, by refusing to classify persons by their diagnosis. By the characteristic of "strange pains" (or simply scarce pains) it is often precisely the difficulty to establish one diagnosis... Then, the "unidentified individual" is not only condamned to be wandered to search their name but also to search a “tribe” which would be theirs. But where is "unidentified individuals’ tribe “?

Without any doubt here, where you can call up until 23h, the time when usually people sleep, but where creativity is just awaking ... Here, where you can find a real listening and understanding, without judgement. And the most important thing (at the risk of repeating myself!) where we can find a perspective.

So let’s this project live and be developped ! And let’s bring our stone to the building and support this initiative.

	"My cyclothymia..." Christian tells ...

"It's been about ten years I was chronically depressed, punctuated by some periods where I could feel almost normal.

Yet there was never any major event that would explain the intensity of the emotional states I was in.
Until now, I didn’t have the instruction manual to manage my life for the good reason that my emotions were always more important than everything else.
I found so increased energy and pleasure during these moments of "happiness" and states of euphoria as much as I missed energy during my "down", when I felt depressed. My social and professional life looked like a discontinuous line, hard to concentrate my efforts to complete a project, follow through an activity
or maintain a friendship or a romantic relationship.

In the last ten years, I have lived in four different places, moved three times in three years in the same city. I told myself that by changing my environment and my acquaintances, I would feel better, that the cause of my problems was external. Only it was always myself that I moved along with my luggage.

It was necessary that my suicidal ideation become recurrent to the point where I no longer thought of the people whom I loved, for me to understand that my sessions of classic psychotherapy provided me only with passive listening, without really being able to translate this into action and manage my life.

Even if they allowed me to better understand certain of my behaviors, they did not explain my profound despair.

I did not refuse life but just the life that I was living : being in a moving elevator, which stopped on the ground floor from time to time, without being able to make it goes up an down, or worse, on a series of roller coasters without being able to get off.

There are about six months, I saw an interview about mood disorders. I recognize myself in the descriptions of mood disorders and it made me feel better to put a name on my illness: bipolar cyclothymic.

I decided then to consult a psychiatrist for looking after myself.

Since then, I have been taking medication in which the doses have been adjusted little by little until the symptoms of depression have faded away considerably.

I felt the effects of the medications after several weeks. I got up in the morning without damning the day to come, I was able to concentrate about three to four hours on my work (before more than one hour would have been an exploit) and, most importantly, I could feel that impulse that allows you to go forwards and even better, to approach others.
I will never be cured of my bipolarity, for the only reason that for me it is not an illness, but more my temperament, as I like to say. I am learning to manage it better and better with the medications and advice.

I hope that these few lines will help you….

Like many bipolar or cyclothymic people, you can have moments of intense creativity. Some people are creative during depression periods when they suffer, whereas the others are only creative during hypomania or mania periods when they are euphoric and excited.

In these creative moments, you can have many ideas, some of which can be very interesting and very good. But there is often no continuation for many people...

Too often, after the fireworks of brilliant but very volatile ideas, there remains nothing or very little, because we don’t note down our ideas and we're moving on another subject. We are the champions of zapping. Many words, many thoughts, but there is only a little action for a short time, that can maintain a sense of worthlessness, non recovery of self-appreciation,marginalization, disconnection with the real world...

LLet ask ourselves the question : Until where the fact of having as many ideas without never developing them, can then cause, maintain, or even worsen, a depression?!

You are fatigued, you feel constantly tired, you fight to stay awake and be able to concentrate, you have memory problems and those around you find that you have changing moods, irritable. These symptoms can also be caused by the syndrome of Excessive Diurnal Sleepiness (EDS) that affects about 11% of the French population. (note 1)

Excessive Diurnal Sleepiness (EDS) is characterized by involuntary drowsiness that happens at completely inappropriate moments during the day (at work, during business meetings, while driving, etc.)

This somnolence can be transitory, secondary to a time difference, or to a voluntary and temporary lack of sleep. On the other hand, once the sleepiness continues for a long time, it can cause real health problems and merits a detailed exploration.

One of the most frequent causes of Excessive Diurnal Sleepiness (EDS) is Obstructive Sleep Apnea Syndrome (OSA) that affects especially middle-aged men who are overweight. This involves repeated obstructions of the soft tissue part of the airway during inspiration, resulting in respiratory arrests (apneas) and a decrease in oxygenation during sleep. The number, intensity and duration of these respiratory arrests, as well the nighttime oxygenation levels, are the parameters that can be measured to establish the severity of the OSA. Many studies have shown that patients with OSA can have cardiovascular complications. It has been shown as well that the number of auto accidents is significantly higher in patients with OSA due to their decreased state of vigilance.

Upper Airway Resistance Syndrome (UARS) is another cause of excessive diurnal sleepiness. This syndrome affects patients with obstructions in the nose or throat that cause a narrowing of the airway. In these patients, the neurological mechanisms allowing for « stabilization » the airway tissues during sleep are effective, preventing the collapse of the soft tissues and respiratory arrests (note 2). Notwithstanding, the effort of getting enough air into the lungs is considerable and is accompanied by micro-awakenings that fragment sleep and make it of lesser quality.

Other causes of excessive diurnal sleepiness exist :

Periodic Limb Movement Disorder, often included with Restless Legs Syndrome, affects about 5% of young adults, and its frequency appears to increase with age (note 3). Two times more common in women, these parasomnias present with involuntary foot movements that occur every 30 seconds and awaken the bedmate and, sometimes, even the patient. These movements are responsible for lightening and fragmenting sleep. The cause is a dysfunction of a cerebral neurotransmitter (striated dopamine). Certain medications used for Parkinson’s disease have shown some effectiveness in this disorder.

Patients suffering from depression or anxiety often sleep a lot during the day, no matter how much they have slept at night. For certain authors (note 4), depression is a cause that is at least as frequent for daytime sleepiness as obstructive sleep apnea syndrome (OSA).

Narcolepsy is a neurological illness characterized by a severe somnolence. It appears as real « attacks » of sleep that the patient cannot usually resist with the patient suddenly and quickly falling asleep, only to wake up some time later. This illness starts in adolescence and thankfully is much more rare than the other causes of EDS. Test your degree of daytime sleepiness by completing the Epworth self test.

	Snoring

Snoring is a respiratory noise which occurs during sleep that is generally not perceived by the patient but which can considerably bother those around him/her.

If in the past it was just a subject for joking, today snoring is a more and more frequent cause for consultation.

On a social level, snoring can be a real problem for couples, forcing the couple to sleep in separate rooms. If its intensity is important, snoring can be very distressing for the entourage, spoiling trips or vacations among friends. Snoring is frequently associated with other obstructive respiratory problems such as Obstructive Sleep Apnea Syndrome (OSA), or Upper Airway Resistance Syndrome (UARS).

It is important to distinguish between « simple » snoring which only affects social life from that associated with OSA or UARS, which represents a real illness with the possibility of causing serious cardiovascular problems. ( 1).

	The mechanisms of snoring

It has been shown that during sleep, normal respiration is through the nasal passages

	1. Under normal conditions, air goes through the nasal passages where it is humidified and filtered, then through the throat by passing behind the palate and tongue, then down the larynx and trachea to get to the lungs. Important gas exchanges happen at the pulmonary alveolar level before the air returns via the same path in reverse. In snorers, many mechanisms cause respiration to happen via the mouth rather than the nose. It has been shown that the air resistance on its way down to the lungs is greater when breathing is through the mouth than when it is through the nose.

	2. Inspiratory muscles must work harder to overcome this increased resistance. In consequence, the speed of the air passing through the throat is higher and friction against tissues is increased. These forces of friction cause the vibrations of soft tissue in the throat. These tissues are those with no underlying bony or cartilaginous skeleton, in other words, the least rigid tissues that vibrate : the soft palate and uvula, the tongue and lateral walls of the throat.

The mechanisms that contribute to snoring are multiple :

a blocked nose increases the force deployed by the lungs to make the air enter the narrowed nasal passages. If the nasal obstruction is significant, the mouth opens and the tissue vibrations appear.

the position of lying on one’s back indisputably facilitates snoring. In this position, the soft palate and tongue fall backwards more easily and therefore approach the back wall of the throat more closely, thereby decreasing even more the caliber of the airway. Air speed will be increased (Bernouli) which will inevitably result in a vibration of the soft tissues in question.

Excess body weight is also traditionally associated with snoring. However, we are finding more and more snoring patients who consult ENT’s who are not overweight. This questions the role of excess weight in snoring. In contrast, the role of excess body weight in Obstructive Sleep Apnea Syndrome is undeniable and will be described in the appropriate chapter. What are the treatments which have proven effectiveness? For a treatment to be effective it needs to first be accepted and followed by the patients. For that, the benefit hoped for should be greater than the constraints of the treatment itself.

	Treatment of snoring

Snoring is a multifactorial problem and needs a rigorous plan of care .Most often, treatment failures are due to an incomplete analysis of the different causes and factors responsible for the snoring. This leads to a maladapted treatment plan which most often leads to failure.

In this way, treating a soft palate without taking into account a blocked nose, or a tongue that is too large generally ends in a disappointing result. As well, proposing a surgical treatment without having tried postural treatment of an adapted hygiene and diet diminishes the chances of success.

Consequently, it appears important to us to adopt a hierarchical and reasonable plan of treatment, taking into consideration all the elements that could be contributing to the snoring.

Several stages should be respected :

Respect a certain lifestyle : light meal without alcohol in the evening, weight loss dietary regime if excess weight

Postural treatment : avoid sleeping lying down on your back. Using certain tricks such as tennis balls sewn into the back of your pajamas or using other aides available in the marketplace that keep the patient from sleeping on his/her back can be very effective.

The effective treatment of a blocked nose is very important. It is better to detect and effectively treat all rhinitis or chronic rhino-sinusitis, a source of persistent inflammation of the mucous membranes and nasal obstruction. In certain cases, medical treatment must be continued daily in the long term.

The obstacle can be located in different places:

At the entrance, at the level of the nostrils: The region of the valve can easily close during inspiration when it has certain malformations.

Nasal septum deviations: They can considerably narrow the nasal air passages especially if they are located at the entrance to the nose.

In the middle part of the nose, the inferior concha: This can be the cause of important obstruction intermittently, with movements or certain positions.

At the level of the throat, many structures should be considered:

Tonsils : If they are very large, they can be a real obstacle to breathing.

The soft palate and the uvula can also be part of the problem is they are excessively long or lack tone.

The tongue that is too large can be a formidable obstacle. The goal of treatment in these patients is to advance the jaw and tongue forwards during sleep to liberate the airway. Individual appliances that are custom-made can be very useful in these patients. Used at night in the mouth, they are usually well tolerated, as long as the nose is perfectly open.

	Upper Airway Resistance Syndrome (UARS)

You snore and you are tired, but you do not have apneas ? This is possible.

The explanation could be the upper airway resistance symdrome – UARS- described in 1992 by Guilleminault and his team at the Stanford Sleep Research Center, California ( 1). These researchers noticed that the diurnal sleepiness of certain patients could not be explained by either obstructive sleep apnea syndrome (OSA) or by a neuro-psychiatric illness.

All of these patients had an important degree of diurnal fatigue due to inferior quality non-restoring sleep. The cause was obstruction of the airway at the level of the nose and of the throat which resulted in repeated micro-awakening, leading to fragmented sleep.

In fact, during sleep, muscles relax and their tone diminishes. It is in these favorable conditions that the abnormal narrowing of the airways favors the appearance of real obstacles at the level of the soft tissues of the throat, which blocks the air from passing into the lungs. This constitutes a respiratory arrest. Fortunately, this arrest is brief, because the brain immediately detects the lowering of oxygen levels in the blood and commands the respiratory muscles to contract and respiration restarts.

This muscular activity triggered by the decrease of oxygen is equivalent to a « little awakening ». Even if its duration is very short, its frequent repetition is responsible for sleep fragmentation. In addition, the effort expended by these patients during sleep to get the necessary amount of air to the lungs is considerable and sleep is of lesser quality.

Are sleepy non-apneic snorers being badly taken care of ?

Is Upper Airway Resistance Syndrome (UARS) not well treated ?

Recent studies have shown that sleepy non-apneic snorers are not being correctly treated.

The first reason is that a good number of these patients escape diagnosis. In reality, because of its serious sequelae, physicians look for obstructive sleep apnea syndrome (OSA) as a priority. Because of this, the principal measurement taken into account by the recordings in the index of apneas and hypopneas per hour of sleep (AHI). In this way, because of their very short duration, the respiratory arrests of the patients with UARS are not counted as real apneas and the measured AHI index is therefore normal. To end up with a correct diagnosis, the measurements should take into consideration other abnormal respiratory events (RERA) that occur during sleep, and especially abnormal inspiratory efforts that awaken. If they are more frequent in a tired patient without obstructive sleep apnea, a diagnosis of UARS is made.

The second cause of the lack of treatment in these patients is linked to the inconvenience of the treatment itself. In effect, physicians often propose continuous nasal airway pressure ventilation (CPAP, or continuous positive airway pressure).

This treatment is very effective in OSA, but it can be very inconvenient and difficult to tolerate for patients with UARS. Essentially, the less the symptoms, less the tolerance for the patients to their treatment apparatus. This would explain why patients with UARS abandon their CPAP treatment more often than those will real OSA. In these latter, the improvement in their quality of life with the disappearance of the fatigue is such that they tolerate the inconvenience of the apparatus more easily than those with UARS.

What treatments have been proven effective ? In all evidence, for a treatment to be effective it must be initially accepted and followed by the patients.

Treatment of UARS (Upper Airway Resistance Syndrome)

As in all other pathology, suggested the treatment in UARS should be adapted to the severity of the illness. The benefit that the patient hopes for should be greater than the inconvenience of the treatment itself. The principle of treatment is to make the obstruction disappear which is responsible for the narrowing of the airway. Two sites should necessarily be taken into consideration : the nose and the throat.

	Dr. Christian Gay

	Hachette Literature (February 20, 2008)

From Publishers Weekly "Loving a Bipolar - I am one - is not within reach of anyone! It is necessary to assess extreme climates, the seasons of the Far North, caving and paragliding. In short, not be afraid of anything, be ready for anything, "says Catherine, letting us glimpse the suffering and happiness, too, there may be to love a manic-depressive. The radical changes in mood caused by this disease are hurting the patient but also those around him: the patient can spend in a short time from deep depression to manic excitement. Spouses, parents, children, during the depressive phases, should take over everything, and during the manic phases, they must exercise tact, diplomacy and patience. Between the two, they live in a state of alert. They also feel a great sense of isolation, and their own reactions to the patient, between empathy and despair, desire of overprotection and rejection, are sources of guilt. The family of a manic-depressive, too often sidelined and uninformed about the disease, yet finds himself on the frontline, and some other endorsing a role nurse or security guard for which they are not trained, causing gender confusion that affects the quality of family relationships. With the perspective to overcome this lack of information that Dr. Christian Gay undertook to write this book: "If, after reading this text, the environment and the patients feel more supported , understood, encouraged, this book will have fulfilled its primary mission. "

Author Bio Dr. Christian Gay is a psychiatrist. Co-founder of France-Depression, it is also the author of several books including Living with ups and downs, with Jean-Alain Génermont (Hachette Literature, 2002)

In 2008, I first met Cecilia when she worked in a store and she spent a difficult period in his life. In talking to her about his career and his passions, I discovered she painted occasionally imaginary characters on the dishes . She had even made a few tableware for the family ... Without more ... One day she agreed to show me some of his accomplishments. I thought it was a good stroke of a pen and especially that it was certainly potential

We got him a stage name: C.cil for her to sign and date his work ... and most importantly, we would have offered participation in several art projects for the collection of drawings different brands.

C.cil only wanted to be and the result was surprisingly fast. Some artistic advice and guidance, and this artist has quickly found a style, a fingerprint.

Although since late 2007, has experienced several episodes C.cil depression which earned him to be milked in (and including today), she also managed to express his talent when his condition permitted, and it has already created 15 paintings beautiful!

Today, in agreement with it, we decided to take the next step ... Tell her early works and help sell them but also to negotiate orders for new designs for international clients.

The Mood Institute is delighted to lend its support to promote her talent, artistic advice, legal support for negotiating contracts, opportunities to publicize and sell their works. We will keep you regularly informed of developments.

The road is still long but finally being able to better express his art to be appreciated and can share it gives her a smile. (below some pictures of his creations)
